(2) An estimated pro rata cash payment of approximately $75 if you did not experience documented losses; this amount may vary depending on the total number of valid claims filed and is not guaranteed. (3) All eligible class members may receive two years of free credit monitoring services, which may include up to $1 million in medical identity theft insurance, monitoring for healthcare insurance ID exposure, medical record number exposure, and unauthorized Health Savings Account spending.
